Adding Surveys & Survey Questions

Effortlessly enhance learner engagement by adding surveys to your courses. Simply navigate to the Add Content button, choose Add Survey, and customise your survey questions to gather valuable feedback
from participants. For more information on this topic, see the below article.
1. Adding a Survey
- To access the survey creation screen select Add survey from the Add Content drop-down list.

Survey Details:
- Name: Provide a mandatory, concise name (up to 255 characters).
- Description: Optional field with formatting options for additional context.

Submit Action:
- Choose an action for when learners submit their survey answers, such as displaying a thank-you message or showing questions with given answers.

Survey Submission Message:
- Define the message that appears upon survey submission, customisable in different languages if needed.

Question Presentation:
- Select the One by one checkbox for each survey question to appear on a separate page.
- Leave it unchecked to present all survey questions on the same page.

Navigation Control:
- Choose Move forward only to prevent learners from navigating back to previous questions. If selected, also choose One by one.
- Select Submit on leave to submit survey answers if a learner abandons the survey, accompanied by a warning message.
- Choose Force answer all to mandate that learners answer every question in the survey.

Display Options:
- Choose List to display question numbers next to each survey question.
- Choose Splash screen to show a survey splash screen to learners before the survey starts. If unchecked, the survey will commence immediately.

Finalising the Survey:
- Click Save and Select Questions to proceed.
- In the Questions tab, view and reuse existing questions or add new ones. Note that changes to original questions will reflect in all surveys using that question.
2. Adding Survey Questions
- You can add a new survey question after creating a survey, or by selecting Go to surveys from the course dashboard and clicking the Questions tab.
- To add a new question, click Add Question and choose the question type required from the list. The add question page displays the required fields based on the question type selected.

- Type: Automatically populated based on the question type selected. You can change the question type using this drop-down list
- Question text: Free text field to enter the survey text. The toolbar provides various formatting options, and allows you to insert tables, hyperlinks, pictures and/or videos. Additional guidance will display below the question text field for certain question types
- Options: Depending on the question type selected, various options may be available (see table for details).
- Once you have completed the fields, click Save Question. You will be taken to the Questions tab where you can view the newly created question and all existing questions.

Free text / file upload | Create open-ended questions whereby the learner enters their answer in a free text field and/or by uploading a file (e.g. feedback in .docx format). | Input type: Select either Text area for free text question only; Upload for file upload question only; or Text area and upload field for free text field with a file upload area. |
Multiple choice – single answer | Create questions with a single answer. The learner selects their answer from a choice of two or more options (up to a maximum of twenty). | Stabilise last option: Check the box to prevent the final option from being randomised |

Reply mode: Select Many checkboxes per row to allow the learner to select multiple checkboxes per matrix row (displayed to the learner as radio buttons). Select One checkbox per row to allow the learner to select only one checkbox per matrix row (if the learner selects a second checkbox in the row, the first checkbox chosen is automatically de-selected). |
